Leave blank if it’s you.
Are they an adult or under 18?
What is the assessment for?
Who will pay for the assessment?Assessments are either self-funded or paid for by the referrer. We send a written quote before anything is booked.
A sentence or two is enough, such as “reading long documents at work”.
We reply in writing.
